If you have problems with the online screening tool, please call the office at 905-877-4111 on the day of your appointment, prior to entering the building, to complete the questionnaire over the phone.
If your results advise you to isolate for any reason, DO NOT COME INTO THE OFFICE. Instead, please contact the office by phone and have your appointment changed to a virtual appointment to ensure we can continue to provide you optimal care. If the doctor determines you need to be seen in person, appropriate arrangements will be made.
